PLEASE SPECIFY IF YOU ARE GETTING A GAS MASK FIT TEST OR A REPIRATORY APR/SAR MASK
**If you are being Initially or Annually Fit Tested on your workplace specific RP, you must have a signed Respiratory Qualification Sheet to verify medical clearance

ALL MASKS ARE FIT TESTED AT BIO- Bldg 6003
**No eating, drinking, or smoking within 30 minutes of your appointment
** Glasses inserts are not necessary

RP
1) Must be clean shaven, facial hair cannot affect seal
2) Fit Tests are required Initially and Annually
3) IAW AFI 48-137, 2.22.3: It is the individuals responsibility to report changes in medical status to their supervisor which may impact their ability to wear a respirator
-Obvious Change in Body Weight
-Dental/Jaw Changes
-Facial or Cosmetic Surgery/Scarring
-New Medical Conditions (ie; Asthma)

Gas Mask
1) Must be clean shaven or meet shaving waiver criteria
2) Gas Masks are ONE time test requirements
3) UNLESS: IAW AFI 48-137, 4.2.5
-10% Body Weight Change
-Dental/Jaw Changes
-Facial or Cosmetic Surgery/Scarring
-New Mask is issued DoD Wide
